         Located in the Public Documents section of your client portal are commonly used forms used by businesses.  At present we are providing the following forms:

    • Form W-4, Employee's Withholding Allowance Certificate (Federal)
    • Form CT-W4, CT Employee's Withholding Certificate
    • Form I-9, Dept of Homeland Security Employment Eligibility Verification
    • Form W-9, Request for Taxpayer Identification Number and Certification
    • MA Form M-4, Massachusetts Employee's Withholding Exemption Certificate
    • NY Form IT-2104, New York Certificate of Exemption from Withholding
    • NY Form IT-2104-E, New York Certificate of Exemption
    • Connecticut Unemployment Separation Package
